1) These are to be compounded in Small Laminar Airflow Hood in IV Clean Room using Equashield Closed System Transfer Device or Clave System.

2) PPE Required:

• Beard cover if required
• Face mask & head cover
• Shoe cover
• HD gown
• 2 pairs of sterile nitrile gloves

3) Place a chemotherapy mat on the Laminar Airflow Hood work surface

4) Use the patient specific program in Pharmacy Keeper for recipe
