NON-ANTINEOPLASTIC OR REPRODUCTIVE RISK
1) Don PPE:
- 2 pairs XT nitrile gloves
- HD gown (chemo)
- Face shield & goggles if there is a risk of splashing or spills
2) Remove bulk bottle or unit dose cup of oral liquid from the designated NIOSH shelf outside the MedCarousel or designated NIOSH shelf by MILT computer
3) Print out MILT labels and gather supplies (Non-sterile chemotherapy prep mat, MILT labels, oral syringes, bottle adaptor or medicine cup, 3 Zip-lock bags, and 2 packs of HD Clean Wipes
4) Place a Non-Sterile chemotherapy prep mat on pharmacy counter. *The non-sterile chemotherapy prep mat will need to be changed with each different medication re-packaged.
5) Place a bottle adaptor on the Bulk oral liquid bottle or pour liquid into a dose cup, then draw up required dose from bulk bottle/unit dose cup/dose cup into oral syringe and cap the end of the syringe. Replace top of bulk bottle with the original manufacturer top for storage. DO NOT LEAVE THE BOTTLE ADAPTOR ON THE BOTTLE FOR STORAGE.
6) Wipe outside of oral syringe and bulk bottle with HD clean wipe 1 and allow the surface of the bulk bottle and oral syringe to remain wet for 30 seconds, then wipe the bulk bottle and oral syringe with HD Clean Wipe 2 and allow the surfaces to dry completely. Placed used wipes and outer gloves in a ziplock bag for disposal
7) Label oral syringes with MILT label and place in ziplock bag for checking
8) Place Bulk Bottle in a ziplock bag for storage on the designated NIOSH shelf by the MILT computer. If a unit dose liquid cjup is used, place used cups in a ziplock bag for checking
9) Clean counter with HD clean wipes and place used PPE, wipes, chemotherapy prep mat in a zip lock bag for disposal in the chemo trash can next to the hazardous medication cart
10) Wash hands with soap and water.
11) Get 2nd check and have another pharmacist sign off in MILT, then placed the bag containing oral syringes in the bin for cart fill or delivery bin for delivery to nursing unit. Extra oral syringes can be placed in the designated blue or red lidded bin on the designated NIOSH shelf by the MILT computer.
12) Any empty liquid bulk bottles, unit dose cups, or expired oral syringes can be disposed in the Black Hazardous Container.
